Disappearing Demon

Was fighting the last demon around Miranda's temple, just recently. (Do not remember type, but was down, north, east from temple itself.)

Was 58th level, so tough fight. Died three times, on third death - 18th blow - used death reset, but when I got back it was gone, no drop. Echoes of fight were still there. Have never seen or heard of simultaneous kill. Was recalled itself?

Am sure i've seen this happen before where you are killed o your last attack and the monster also dies.
