Unable to Obtain Profession Form

I'm running the leveling challenge for Alf. He has the Scepter of Death in inventory. I've clicked "use" with Randahl and cannot get a form. It just says, "Alfred Elpin has not correctly performed the task needed to acquire a profession form!"

If I recall, I can get the form at any time prior to 15. But, I cannot use it until level 15.

You will have to wait until level 20 to obtain the form.
